Our practice has developed an adult dental savings plan to give you optimal access to the care and treatment you need and deserve. No hidden clauses, no restrictions – just quality care for one low price.

In-Office Annual Dental Savings Plan

We offer adult, child and periodontal membership plans to meet your needs and circumstances. Your one-year membership starts on the day your investment is paid in full.

Adult Membership Plan 

$349.99 Annually

Your membership plan includes:

  • 2 Exams (D0150, D0120)
  • 1 Limited Exam, X-rays Included (D0140)
  • 2 Regular Cleanings (D1110)
  • 2 Fluoride Applications (D1208)
  • All Necessary X-Rays
  • 10% Discount on All Other Dental Services
  • $500 Off Clear Aligners

Child Membership Plan 

$300 Annually

For children age 12 or younger at the date of enrollment.

  • 4 Periodontal Cleanings (D4910)
  • 2 Fluoride Applications (D1208)
  • All Necessary X-Rays
  • 20% Discount on Scaling and Root Planing (D4341)
  • 10% Discount on All Other Dental Services

Additional Information 

  • This is a dental discount plan, not dental insurance. It cannot be combined with any other dental insurance, discounts or promotions.
  • The Dental Savings Plan is only valid at Smiles On Hampton, and is not transferrable to other offices or to specialists.
  • The effective date of this plan is the day you sign the agreement. This plan does not auto renew.
  • Exams, X-rays, cleaning sand fluoride must be performed within the year of enrollment. Benefits do not carry over to the next year.
  • Payment is due at the time of service for any work not covered 100% by your plan.
  • Payment of less than the plan’s duration membership fees may result in being charged usual and customary fees for all services (including those already provided) and being charge the remaining months’ fees in a lump sum.
  • Should there be dental treatment needed following any type of injury where a lawsuit and therefore outside medical care, disability or workers comp type insurances are involved, this discount plan cannot be used.
  • Your plan is non-transferrable. Family members cannot be substitutes for another family member.
  • This plan is non-refundable. Refunds will not be issues if a patient chooses not to use their dental plan.
  • Members must maintain membership throughout the duration of clear aligner treatment.
  • This plan qualifies you for dental services only. Products are not included in your plan.
  • A fee (based on the length of your appointment) may be incurred for each broken appointment without a 24-hour advance notice.
  • Rates are subject to change annually.
